OVERVIEW
HCSIC is seeking a Community Support Specialist to join our team and play a key role in advancing the quality, coordination, and sustainability of Home and Community Support Services across Northeastern Ontario.
The Community Support Specialist will support and lead regional initiatives, foster collaboration among service providers and partners, and contribute to project management, data analysis, stakeholder engagement, and strategic coordination.
This position will establish and maintain strong working relationships with Health Service Providers and regional partners and will play an important role in supporting large-scale operational and system improvements across the Community Support Services sector.
KEY RESPONSIBILITIES
Strategic and Sector Support
- Provide support to regional working groups and initiatives, ensuring activities align with identified strategic priorities.
- Monitor and advise on emerging sector trends, opportunities, challenges, and best practices.
- Support advocacy and sector development initiatives that strengthen Community Support Services across Northeastern Ontario.
- Provide information, resources, and recommendations to support informed decision-making and system planning.
Operations and Project Management
- Lead and support the implementation of regional initiatives, projects, and quality improvement activities.
- Conduct environmental scans, research, data analysis, and capacity planning to inform regional decision-making.
- Oversee the Northeast CSS Network on Caredove, including user onboarding, quality monitoring, stakeholder support, and outreach education.
- Develop and maintain project plans, timelines, tracking tools, and reporting mechanisms.
- Monitor project progress and ensure deliverables, milestones, and outcomes are achieved and appropriately reported.
- Identify risks, barriers, and opportunities and develop recommendations or solutions as appropriate.
Collaboration and Knowledge Sharing
- Facilitate regional meetings, learning sessions, communities of practice, and other opportunities for knowledge exchange.
- Develop and disseminate newsletters, resources, research updates, policy information, and innovative practices through appropriate communication channels.
- Support the coordination of regional interRAI education, including education, resources, and competency evaluations for CSS assessors.
- Promote collaboration, knowledge sharing, and the adoption of evidence-informed practices across the sector.
Stakeholder Engagement and Advocacy
- Develop and maintain strong, collaborative relationships with Health Service Providers, Indigenous and First Nations communities, government partners, and other key stakeholders.
- Organize and facilitate consultations, workshops, planning sessions, and other engagement activities.
- Represent HCSIC and the Community Support Services sector at regional and provincial meetings, committees, and discussions.
- Act as a resource and point of contact for HSPs on regional initiatives, projects, and sector-related matters.
Equity, Diversity, and Inclusion
- Ensure regional initiatives and projects reflect the diverse needs, experiences, and priorities of Northeastern Ontario communities.
- Champion culturally safe, inclusive, and anti-oppressive approaches in planning, engagement, and service improvement.
- Support initiatives that address barriers to equitable access to Community Support Services.
QUALIFICATIONS
Education and Experience
- College diploma or bachelor’s degree in health administration, public policy, geriatric care, or equivalent experience.
- 2–5 years of experience in community support, healthcare coordination, or project management.
- Experience working in a multi-stakeholder environment, preferably within the nonprofit or public sector.
- Experience coordinating projects, facilitating meetings, conducting research or analysis, and developing reports or recommendations.
Skills and Qualifications
- Strong analytical, planning, and facilitation skills.
- Proficiency in data analysis and reporting tools (e.g., Excel, Power BI, Electronic Medical Records).
- Exceptional communication and interpersonal abilities.
- Demonstrated ability to lead initiatives, manage multiple priorities, and meet deadlines.
- Ability to interpret information, identify trends, and translate findings into practical recommendations.
- Knowledge of Ontario’s Home and Community Care and CSS systems.
- Knowledge of relevant provincial legislations, policies, and regulations.
- Bilingualism (English/French) is an asset.
WORKING CONDITIONS
- Full-time, permanent position based in Sudbury, ON.
- Standard office environment with frequent use of computers, with a strong emphasis on virtual meetings and collaboration.
- Hybrid and/or virtual collaboration with regional partners may be required.
- Occasional travel throughout Northeastern Ontario is required for regional meetings, consultations, training, and project activities.
- A valid driver's licence and access to reliable transportation may be required.
