Stay Home Forever Inc. (SHF) is a growing home and community care organization dedicated to helping individuals remain safely, independently, and comfortably at home. We provide high-quality, client-centered services through an integrated team of Personal Support Workers, Nurses, and Allied Health Professionals.
As our organization continues to grow, we are looking for an organized, adaptable, and service-oriented Coordinator to join our Operations team.
Position Summary
The Coordinator plays a critical role in the day-to-day coordination of community healthcare services. Working closely with clients, families, referral partners, leadership, and frontline staff, the Coordinator is responsible for scheduling, service coordination, responding to operational changes, and supporting the delivery of safe, efficient, and client-centered care.
This is a full-time, in-office position based in Markham, Ontario.
What You'll Bring
Education
- Post-secondary diploma or degree in Healthcare Administration, Health Sciences, Business Administration, Social Services, or another related field is preferred.
- Equivalent combinations of education and relevant work experience will be considered.
- Healthcare education obtained outside of Canada will be considered. Internationally Educated Healthcare Professionals (IEHPs) are encouraged to apply, provided they meet the experience requirements outlined below.
Experience
- Minimum 1–3 years of experience coordinating or scheduling frontline healthcare providers in one or more of the following settings:
- Private home care
- Publicly funded home and community care
- Community support services
- Healthcare organizations
- A comparable healthcare environment.
- Experience coordinating Personal Support Workers (PSWs), Registered Practical Nurses (RPNs), Registered Nurses (RNs), therapists, or other frontline healthcare professionals.
- Experience managing multiple priorities in a fast-paced environment while maintaining exceptional customer service.
- Demonstrated ability to make sound decisions and adapt quickly to changing operational needs.

Technology Skills
The successful candidate will:
- Be proficient with Microsoft Office, including Outlook, Word, Excel, and Microsoft Teams.
- Be comfortable working with multiple software applications simultaneously.
- Have experience using scheduling software, Electronic Health Records (EHRs), or workforce management systems, or demonstrate the ability and willingness to quickly learn new technologies.
- Adapt confidently to evolving digital workflows and operational systems.
Experience with platforms such as AxisCare, AlayaCare, Procura, GoldCare, or similar healthcare software is considered an asset.
Primary Responsibilities
The responsibilities listed below represent the core functions of this role. As Stay Home Forever continues to grow, responsibilities may evolve to meet operational and organizational needs.
Responsibilities include, but are not limited to:
- Coordinate daily schedules for frontline healthcare providers.
- Respond promptly to schedule changes, employee call-ins, client requests, and urgent staffing needs.
- Match client service requirements with employee skills, availability, continuity of care, geographic location, and organizational priorities.
- Communicate professionally with clients, families, referral partners, and healthcare providers.
- Monitor schedules throughout the day to ensure efficient service delivery and proactively resolve scheduling conflicts.
- Maintain accurate documentation within organizational systems.
- Collaborate with operational and clinical leadership to support safe, effective, and client-centered care.
- Escalate operational or clinical concerns in accordance with organizational policies and procedures.
- Participate in quality improvement initiatives and support ongoing operational excellence.
- Maintain confidentiality and comply with all applicable privacy legislation, organizational policies, and professional standards.
