Looking for a new and exciting job opportunity? We are looking for a Manager, Complex Special Needs to join our Halton Support Services team! In this position, you will be responsible for the operational oversight and coordination of supportive living placements for children, youth and adults receiving services through Halton Support Services (HSS) program, with a primary focus on individuals placed in Outside Paid Resources (OPRs). Working as the designated Placing Agency, the Manager ensures placements are safe, appropriate, fiscally responsible, and responsive to each individual's changing needs.
What You Will Do
This position provides day-to-day supervision to one (1) staff while independently managing a specialized portfolio of supportive living placements across a broad geographic area. The Manager coordinates placement planning, completes monitoring visits, oversees service agreements and Ministry requirements, prepares individualized funding submissions, monitors budgets, and works collaboratively with families, community agencies, and internal and external service providers to promote positive outcomes and placement stability.
Operating with a high degree of autonomy, the Manager acts as a key liaison between individuals, families, Outside Paid Resources, community and internal CWSDS partners, and the Ministry of Children, Community and Social Services, ensuring legislative, contractual, and organizational requirements are met while supporting quality services and reducing the risk of placement disruption.
1. Outside Paid Resource (OPR) Oversight
- Coordinate the identification, assessment and selection of appropriate Outside Paid Resources (OPRs) for children, youth and adults requiring supportive living placements
- Develop placement profiles in collaboration with individuals, families and community partners to support successful matching with service providers
- Coordinate admissions, transitions and ongoing placement planning
- Conduct regular monitoring visits to OPRs to assess service quality, health and safety, and the ongoing suitability of placements
- Monitor placement stability and work collaboratively with OPRs and families to resolve concerns to reduce the risk of placement breakdown
- Ensure insurance documentation, service agreements, consent documentation and other required records remain current and up to date
- Recommend changes to placements or services to the Clinical Manager of Halton Support Services and Director, Community Programs, as appropriate
- Coordinate transitions as individuals move into adult developmental services if a funded vacancy becomes available
2. Service Coordination
- Provide complex service coordination for individuals requiring specialized residential supports
- Facilitate case conferences involving families and multiple service providers
- Support families in navigating developmental services, health care, education and community resources
- Collaborate with hospitals, schools, MCCSS, Developmental Services Ontario, police and community agencies to coordinate support
- Participate in community planning tables and regional committees, including the Situation Table
3. Ministry Compliance & Quality Oversight
- Ensure compliance with MCCSS requirements related to supportive living placements
- Complete Serious Occurrence Reports (SORs) within mandated timelines and alert Director, Community Programs of Level 1 SORs
- Monitor services delivered by Outside Paid Resources to ensure individuals receive safe and appropriate supports
- Maintain accurate documentation within organizational databases
- Monitor placement documentation to ensure organizational and legislative requirements are met (e.g. up to date consent forms)
4. Financial Administration
- Prepare individualized funding requests and annual budget submissions for community committee endorsement and then for MCCSS, in a timely manner
- Monitor individualized budgets and expenditures in collaboration with Finance, Director, Community Programs, and Accounts Payable, at CWSDS
- Review invoices and identify discrepancies
- Close collaboration with the Clinical Manager of Halton Support Services regarding budget utilization and funding requirements
5. Supervision
- Provide day-to-day supervision, coaching and guidance to one (1) staff
- Support orientation, onboarding and training of staff and students
- Participating in recruitment and selection activities
- Support performance management
6. Community Partnerships
- Develop collaborative relationships with community agencies and service providers
- Use a coaching style to support cross-sectors learning about the Developmental Services (DS) sector
- Participate in regional planning initiatives and committees
- Promote coordinated service delivery across developmental services, education, health care and justice sectors
WHAT QUALIFICATIONS AND SKILLS DO YOU BRING?
Education
- Bachelor's degree in Social Work, Social Services, Human Services, Psychology or a related discipline
- A diploma in a related field combined with significant relevant experience will be considered
Experience
- Minimum five (5) years' progressive experience supporting individuals with developmental disabilities and/or complex needs
- Experience coordinating residential placements and community-based services
- Experience working collaboratively across multiple service systems
- Previous Manager experience is considered an asset
Knowledge
- Knowledge of Ontario's developmental services system
- Knowledge of MCCSS requirements related to residential services and funding
- Knowledge of residential service delivery models
- Knowledge of person-directed planning principles
- Knowledge of risk assessment and crisis management
- Knowledge of community resources throughout Halton and Central West Ontario
