Physician Assistant – Franklin FHO (Monark Medical Clinic)

(40 hours/week, Permanent)

Langs is an organization with deep roots in community development and a proven commitment to improving access to care for those most in need. Building on decades of trusted, community‑responsive service, Langs now serves as the lead agency for the Cambridge and North Dumfries primary care expansion funding. Working in close collaboration with the Cambridge and North Dumfries Ontario Health Team, the Primary Care Network, and local Family Health Organizations, Langs is helping to advance a coordinated, system‑wide approach to primary care. Together, we are focused on strengthening access and continuity of care by attaching more than 9,000 patients to primary care within the next year—responding directly to the evolving needs of our growing community.

What We are Looking For:

We are currently recruiting for one Physician Assistant to provide primary care services within Franklin Family Health Organization (FHO) (Monark Medical Clinic). Physician Assistants (PAs) are highly skilled healthcare professionals who work collaboratively with physicians and other members of the healthcare team to deliver comprehensive patient care within a delegated medical model. The PA will support physicians in the provision of primary care services to individuals and families across the lifespan, with a strong focus on improving access to care, chronic disease management, preventative care, and serving vulnerable populations.

A successful candidate in this role would have

· Graduate of an accredited Physician Assistant education program recognized in Canada or the United

· States

· Current certification as a Canadian Certified Physician Assistant (CCPA) or Physician Assistant-Certified

· (PA-C)

· Eligible for registration with the College of Physicians and Surgeons of Ontario (CPSO) as a Physician

· Assistant

· Minimum 2 years’ experience in a primary care or community-based setting preferred

· Experience working with complex patients and chronic disease management

· Experience working within an interdisciplinary healthcare team

· Strong communication, assessment, organizational, and clinical reasoning skills

· Clear understanding of the Social Determinants of Health

· Ability to work collaboratively within delegated medical directives and physician supervision models

· Experience in program development, implementation, monitoring and evaluation considered an asset

· Must be available to work evenings as required

In this role you will be expected to complete the following tasks

· Conduct patient interviews, obtain medical histories, and perform physical examinations

· Assess, diagnose, treat, and monitor patients within the delegated scope of practice established with

· supervising physicians

· Order and interpret diagnostic tests and investigations as delegated

· Develop and implement treatment and management plans in collaboration with supervising physicians Provide patient education, counselling, preventative care, and health promotion services

· Perform procedures and interventions within delegated authority and organizational policies

· Contribute to chronic disease management, preventative screening, and episodic care initiatives

· Participate in chart documentation, case conferences, quality improvement initiatives, and chart reviews

· Collaborate effectively with physicians, nurses, nurse practitioners, allied health professionals, and

· community agencies

· Apply evidence-based practice and clinical guidelines in the delivery of patient-centered care

· Participate in the development, implementation, monitoring and evaluation of programs and services for

· individuals, families and the broader community

· Apply and participate in the development of appropriate professional and organizational policies, protocols

· and procedures

· Understand the role of, refer appropriately to, and communicate effectively with other health professionals

· and community services

· Participate in the development and maintenance of health resource materials

· Other duties as assigned

The successful candidate would be eligible for the following benefits pro-rated to hours worked:

· Salary of $83,987 Annually

· 3 Weeks paid vacation, with an increase to 4 weeks after 1 year of employment

· 15 Sick/Personal Days per year

· 10 Professional Development days per year

· Enrolment in Healthcare of Ontario Pension Plan (HOOPP)

· Comprehensive Extended Health and Dental Care Plan including a $1000 Healthcare Spending Account.
